Eligibility Requirements: Homeless, at-risk, 18 years of age or over, diagnosis of psychiatric disorder, history of hospitalization or difficulties.
Intake Procedure: Referrals will be accepted from any source, majority are recruited by outreach efforts of the staff.

About the Program: The program objective is to make available and accessible an appropriate array of services to persons who have a mental illness and are homeless or at imminent risk of becoming homeless to facilitate placement into housing. The program has an aggressive outreach component geared toward assessment and linkage to appropriate treatment services, and toward eliminating the barriers to housing by providing psychiatric evaluation, medication, counseling, referral, advocacy and case management. It is not unusual for the case manager to have to make several contacts with an individual to win their trust and confidence to even make a preliminary assessment of the individual's needs. The program works closely with the Sine Domus Primary Health Clinic and with other providers serving persons who are homeless to increase access to services.
